David Grant USAF Medical Center - Pathology

DGMC's Pathology Services -- also referred to as Clinical Laboratory & Anatomic Pathology -- is part of the 60th Diagnostics and Therapeutic Squadron.  It is one of the largest medical laboratories in the Air Force, performing more than two million tests annually.

Laboratory Test Ordering: All laboratory analyses requires a written order from a licensed healthcare provider. Orders from 60th Medical Group Providers must be placed into the Composite Healthcare System (CHCS) prior to the patient's arrival in the lab.

Hand-written orders: These must be properly completed to include:

- Patient's full name (last, first, middle initial);
- Full SSAN, including family member prefix (FMP);
- Rank/status;
- Home/duty phone;
- Date of Birth;
- Test(s) requested;
- Appropriate clinical data (see specific sections for details), and;
- Requesting provider's full name
- Provider's signature

Positive Patient Identification: A positive ID is crucial to ensure test results are placed in the proper patient record. Patients should be prepared to present a government issued photo ID card.
