DGMC's Admissions & Dispositions department has two convenient locations within the facility that are available 24/7/365 for your patient administrative needs:
 
-- Patient Service Center (PSC), located at Window #3 on Level 2  in the area known as "The Patient Mall," and; 

-- DGMC's Emergency Department for after normal duty hours, weekends and holidays.

Areas that A&D are responsible for include:
Patient Registrations - Composite Health Care System (CHCS)
Routine Admissions & Discharges
Expectant Mother Pre-admissions
Pre-Admissions (All to include VA Patients)
Processing of Deceased / Funeral Facility coordination
Health Insurance Portability and Accountability Act (HIPAA) 
Security of patient valuables
Lost & Found

For patient registrations:
Please click the following link to download and open the CHCS Registration Form (.pdf format, requires Adobe Reader). The form can be filled out in advance by the patient and hand carried to the A&D office or Emergency Department (if after normal duty hours, weekends or holidays) or faxed to the number listed at the top of the form.

A&D's normal hours of operation are Monday through Friday (excluding weekends, holidays and after hours*) from 7:30 a.m. to 4:30 p.m.

*NOTE: For weekends, holidays and after normal duty hours, please contact A&D personnel located in the Emergency Department at 423-3824 concerning any of the above A&D areas of responsibility.

**Patients with pre-scheduled admissions prior to 7:30 a.m. should report to the Emergency Department to be admitted.
