Hospital makes medical records available

Did you know it is easier than ever to obtain copies of your medical records? David Grant USAF Medical Center's release of information department is now offering copies of medical records via compact disc. The 60th Medical Group Form 101, Authorization for Disclosure of Medical or Dental Information, must be completed before medical information can be released. 

Requests may be accomplished at the DGMC patient service center located on the second level next to pharmacy or with the medical records department on the first level. Information in the medical record cannot be released without prior written authorization from the patient or his/her legal representative.

All ROI requests are processed daily and prioritized according to patient care requirements. We process immediate patient-care requests, retiring or separating active-duty service members and continuity of care requests, respectively. All others are processed according to the date received and completed within 30 days of the request.

One complete electronic medical record per patient will be provided free of charge on a locked CD. Instructions on how to copy the information from the CD to a personal computer and how to make additional CD copies are provided. Please note, when submitting a claim to the Veterans Administration, only a labeled, locked copy will be accepted for this purpose. Additional CDs will assess a $25 copying fee.

Dental and mental health records or digital images from radiology must be requested directly through these departments as they handle their own ROI requests. The 60th MDG Form 101 must be completed for each request.
