As of Nov. 1, active duty members are able to schedule their annual personal health assessment appointments through David Grant USAF Medical Center's Central Appointments desk. Call 707-423-3000 and press option "1", followed by option "4" to speak with an appointment representative.

"This new process should improve the effectiveness and efficiency of the PHA appointing process," said Technical Sgt. James Bailey, 60th Medical Operations Squadron noncommissioned officer in charge of the PHA office. "Previously, the only way to schedule a PHA appointment was by contacting the PHA office."

Completion of the Air Force web-based health assessment, which can be found on the Air Force Portal under Air Force links to the Health and Fitness Tab, is still required before making a PHA appointment.

"It is the responsibility of every active-duty member to check with their unit deployment managers and call to schedule their PHA appointment, as well as maintain their world-wide duty status," Tech. Sgt. Bailey said. "PHA is a tool to help monitor mission-ready status, therefore it's vital to the active-duty member for assessing and accomplishing preventative health needs."

The PHA is also valuable for squadron commanders in determining unit readiness to fulfill deployment requirements. If you have any questions about PHA appointments or other PHA processes, please contact the PHA office staff at 423-5460.
