David Grant USAF Medical Center - Referral Management

David Grant USAF Medical Center's Referral Management provides "one-stop shopping" to our patients. We are located on Level 2 (2nd Floor) in the Patient Service Center as part of "The Patient Mall" in DGMC, next to Pharmacy and the Clinical Labs.

If your civilian provider has referred you to one of the following clinics, utilizing your military healthcare benefits at DGMC promises no outpatient co-pays, and the integrity, service and excellence you deserve:

General Surgery
Heart, Lung & Vascular Center
Orthopedics
Radiation Oncology

If your Primary Care Manager writes a referral for you to be seen in a Speciality Clinic, you will be asked to visit Referral Management to have that appointment scheduled before you leave DGMC.  Our Referral Management staff is committed to assisting you with the coordination process.

Appointment Availability: Appointment schedules open as far as four weeks in advance, but availability can be limited by staffing (see below)

Care to Civilian Facilities: If services are unavailable at DGMC, Referral Management staff will fax your consult/referral request to United Healthcare Military and Veterans (UHC), our TRICARE  contractor, for referral to a TRICARE network civilian facility.  You should recieve an authorization letter in the mail 7-10 business days after UHC processes your referral.  The letter will indicate the name of the civilian facility/provider, phone number to call to schedule your appointment, and start/end date of the authorization.

FOR CIVILIAN HEALTH CARE PROVIDERS ONLY:  DGMC has an Admissions Call Center for Health Care providers at (707) 423-7790.    Staffed 24 hrs a day, 7 days a week,  this operation has been designed to provide dedicated administrative support to facilitate timely admission decisions and smooth patient transfers between DGMC and other medical facilities.
